This privacy policy applies to personal information collected by Dental Wellness (DW)
Notice: ‘we’, ‘us’, ‘our’ ‘DW’, refers to Dental Wellness which will be referred to as DW throughout this document). ‘you’ and ‘your’ refers to individual patients or clients of DW.

· details from health professionals you have previously been a patient of;

· records of contact with DW such as system notes
· Basic statistical information for business development purposes including service development and risk assessment

o medical history;
o family medical history;
o current health issues;

4. How Information May Be Collected

· If you choose to provide personal information via completing new patient forms on our website.
· When you complete forms, send us an email, provide information to a staff member during a visit to our DW practice or by telephone;
· If you choose to provide personal information directly to a DW dental professional whilst receiving dental services at our practice.

· we may collect information from third parties such as Medicare or your health insurance provider or;
· we may collect information from other health practitioners, (dentists and dental specialists); (ie if you were referred to us by another dental practitioner).
If you have provided DW with information about another person, it is your responsibility to inform them that you have done so and that they should refer to this document for details regarding how we will handle their personal information and their right to access their information.

5. Use and disclosure of Your Personal Information

Your personal information is primarily used for administrative processes that enable safe and efficient product and service delivery to you. Where required, we may disclose your personal information on a confidential basis to associated staff members within the practice. Your personal information may be used for any of the following purposes:

· The provision of dental services and products to you;
· The administration and maintenance of your client files at our practice;
· To send correspondence to you in relation to the practice and any dental services or products being provided to you
· To answer any enquiries you may have related to your dental care here at DW;

· To administer your invoicing for dental services, credit card payments, or to collect unpaid invoices;
· To process your Medicare and health insurance claims;
· To contact you for follow up regarding further dental examinations or treatments that may be required;
· To resolve client complaints;
· To comply with any laws or regulations.
If you submit online forms or send emails to us containing personal information, we will use that information to respond to your enquiry or to contact you directly. The main purpose of collecting and using personal information submitted through our website is to respond to DW client enquiries regarding our services. We may store your contact information for future reference.

· Your contact information may only be disclosed to health insurance companies or third party health benefit providers to assist in the processing of a claim for reimbursement or payment/co-payment of dental treatment submitted by you or on your behalf;
· Where disclosure is required by law, (eg. compulsory notices from courts of law, government agencies or tribunals).

To ensure your privacy is respected and maintained, we will take reasonable measures to ensure that your personal information is stored securely and is protected from misuse, loss and from unauthorised access, alteration or disclosure. DW maintains strict data security standards and procedures that are in accordance with Australian privacy laws including the Australian Privacy Principles.

Your personal information may be used by DW staff for administration and in the provision of services to you. We will store your personal information for as long as it is required to provide these services and products to you and to comply with all associated legal requirements. If we no longer require your personal information, we will take reasonable measures to securely destroy or de-identify your personal information except in circumstances permitted or required by law.

You have the right to access personal information that is held about you by us. You can at any time, ask us for access to the information that we hold about you. In order to maintain high quality customer service and practices it is essential that the information we hold about you is accurate and up to date. At DW, we will strive to ensure that your personal information is accurate, complete and up to date at the time of collecting and also when using or disclosing it. If you believe any information we hold about you is inaccurate, incomplete or out of date, you should contact us to let us know so we can amend your records accordingly.

Protection and maintenance of personal privacy is a priority for us. Our website does not provide facilities that guarantee secure transmission of information over the internet. The before and after photographs on our website are from actual dental procedures on patients at our practice. No retouching or other modifications have been made to any the photographs. All patients who appear in these photographs have given consent and no photos are displayed without their written or verbal consent.

As an ever evolving practice we reserve the right to change the content or services found on the DW website without notice. Consequently our Privacy Statement may change at any time also without notice.
